I see my p-doc tommorrow, Been on Lithium 600 mg since the 24th of Ocotber. Feel worse than ever. Very irritable. and spaced out. Been on 40 mg of Paxil for 13 years. Could the Paxil be causing my mixed state? I have to eat Clonazepam to maintian, 1-2 mg, per day. Feel more unstable than ever. Have major stressors at home. Please answer, Lynn |

Hi Lynn,
I know how difficult it can be when you have things coming at you from all angles, and trying to make sense of whats causing what. Sounds like you have extra stress you don't need too. Good that you have a PDoc review tomorrow, good luck with that. Worth writing a list of the important Q's/points you have before the app't as a reminder.
med. changes are a difficult time too. The experience with lithium in my family is that it usually took months to get levels stabilised, and to see an improvement in mood too. Give it plenty of time.
I know that paxil and other antiD's can trigger episodes when starting out, but you've been taking for so long now, be guided by your dr. (its one to adjust dose slowly from what i read). SSRIs have been good as treatments for me with anxiety/mood.
Counselling and/or supportive therapist might be helpful now for you as well. |

There is a body of good research that suggests that long-term use of antidepressant medication can increase the incidence of mixed phases or rapid-cycling in Bipolar I Disorder.
Here is a recent study.

The treatment of bipolar depression requires the resolution of depression and the establishment of mood stability. A basic problem is that the treatments used in treating bipolar depression were developed and proven effective for other disease states: antidepressants for unipolar depression, and mood stabilizers for mania. The panel addressed four unresolved questions regarding depression in relation to bipolar disorder: (1) the relative effectiveness of different antidepressant treatments; (2) the relative likelihood of mood destabilization with different antidepressant treatments; (3) the effectiveness and role of mood-stabilizing medicines as antidepressants; and (4) the optimal approach to mixed states. The selection of an antidepressant depends both on its relative lack of mania- or hypomania-provoking potential and on its effectiveness against bipolar depression. There is little definitive evidence distinguishing effectiveness of the major groups of antidepressive agents, so side-effect profiles and pharmacokinetics are major considerations. The underlying bipolar disorder should be treated with mood stabilizers started simultaneously with any antidepressive treatments. Lithium, divalproex sodium and carbamazepine have all been found to be helpful, to some extent, in treating bipolar depressive episodes as well as for long-term mood stabilization. There is little evidence for long-term benefits of antidepressive agents in bipolar disorder, and some evidence that they may destabilize the disorder. Therefore, in contrast to the long-term use of mood-stabilizers, antidepressant use is recommended on a temporary basis. The duration of antidepressant treatment is determined by past history in terms of liability for mood destabilization, and by the ability of the patient to tolerate gradual antidepressant discontinuation without return of depression. Mixed states, where symptoms of depression and mania coexist, are regarded as a predictor of relatively poor response to lithium, and divalproex has been found to be more effective. Carbamazepine may too be useful in mixed states. Most patients with mixed states in actual practice require combinations of mood stabilizers, though there is little controlled data regarding such co-prescription, especially from a long-term perspective.

I was on Paxil when i was 16. It made me suicdial and i ended up in the hospital. Im not saying thats what it does but there could be a possibility.
although some meds are tested with side affects and it may not have happened in other people. We have to remember that our genetic makeup is all different. So meds work on us differently
Life & Death Energy & Peace If i stopped today. It was fun. Even the terrible pains that burned me & scarred me. My soul is worth it for having been allowed to walk where i've walked. Which was to hell on earth Heaven & earth back again,into,under, far inbetween, through it, in it, far and above it!

Just called my pdoc, I see him on tuesday at 1pm. He advised me to stop taking the Lithium, immediately. so, I flushed them. I was told to stay on the Paxil for now. I am afraid, but feeling more hopeful. Thank you Polarone Chevy and Dr. Long for your support ! Lynn in Toronto

Okay, just read a ton of info on Paxil thanks to Dr. Longs site, and long term use is not advised. If it is used long term low doses are recommended. I've been on 40 mg for 13 years. (my own fault for not going for follow ups) PS- paxil can cause depression I know for sure that i am depressed. Will be so glad to get this cleared up. Lynn

Still figuring my way around the site. I saw Psychiatrist. Was put back on Lithium 600 mg at nite, 40 mg of Paxil each morning, and 0.5 mg of Clonazepam as needed. Max 3 per day. It was the shakey feeling I didn't like on the lithium, figured that out after reading Sheerlins posts. (TQ BTW, SL) I'm oh so new to this dis-order, I had thought I was coping well before, my assessment and dx. BUT shall procede with treatment. Hoping to sleep soon, took 4 clonazepam, and still nothing, wound up like an 8 day clock. See Doc again in January. PS- Docotr kept meds the same as the Paxil were working for the panic attacks, the clons I am used to, and the lith, he wants me to try awhile longer. Where's the straight jacket icon ?

Up date: March 2009. Dx'd as BP 1 in June 2008. dropped to 20 mg of Paxil, increased to 900 mg of Lithium, and had an add on Anti psychotic, (seroquel) to help with sleep, and paxil wd's. Long story short, I fought the Lithium increase but in the end gave it a good try, and find it helps balance mood. Am entering change of life, (ha,ha) and find I still go bonkers during a pre em cycle. Not pretty at all. Still have stressors, but am trying to look at them in a positive light. Lost the love of my life, in October, extreme stressor. Still grieveing, each month is a trigger, On a personal note, which may trigger, he was found, 6 days after his death, we had no closure, no service, many questions unanswered. about his last moments. Sorry, I know this may upset some, but have to share. After all of the research we have done, we surmise, he fell, and bled out, while in a drunken state. Forensic cop brother thinks he probably went easy. Just curled up and went to sleep. This has been the largest, blow to my sanity since my mothers near death from a slow heart rate, which was corrected by a pace maker. I miss him, very much, and find great comfort in visiting the Cemetary where his ashes are scattered. This resting place is a mere walk from where I live. We lived a short distance apart from each other, and kept in touch . by phone, and not enough visits. Doc wants me to drop Paxil to 10 mg. which I will, a bit afraid of depression coming back though. I wonder now, If I am going to be dependant on Seroquel the rest of my life? I wonder if I will be med dependant the rest of my life? Seems to be the answer for now.
Note to self: ask God what His plans are, for me, for the day. . |
